- Home-based long-acting injectable medications
- Home-based medication-assisted therapy for substance use disorders
- Care coordination with community supports
Chronic disease management
With your guidance, we support goal-directed treatment and improve your patient’s ability to self-manage common chronic conditions like CHF, COPD, asthma, diabetes, and hypertension.
Care coordination
We ensure that your patients have everything they need to stay healthy and independent at home. We assist with durable medical equipment, referrals to home health agencies and adult day programs, and more. We also navigate through prior authorization processes.
Support for social determinants of health
We ensure that your patients get all the benefits they qualify for. For example, we:
- Help apply for benefits like SNAP and food stamps
- Find housing and fill out the paperwork
- Connect patients with programs to lower gas, electric, and internet bills
- Set up with rides to get to appointments
